do declare on oath that my personal description is:

height 5 feet 9 inches, weight
135 pounds, color of hair brown,
color of eyes grey, other visible distinctive
marks none
I was born in Zombor, Hungary on the 18th
day of September, anno Domini 1906; I now
reside at 9503 Avenue N, Brooklyn, New York.
I emigrated to the United States of America from Hamburg,
Germany on the vessel Oropesa; my last
foreign residence was Bajn, Hungary; I am not
married; the name of my wife is _____; she was born at ____ and now
resides at ____.
It is my bona fide intention to renounce forever all allegiance and
fidelity to any foreign prince, potentate, state, or sovereignty, and
particularly to The present Government of Hungary and (or) The
Kingdom of the Serbs, Croats and Slovenes, of whom I am now
a subject; I arrived at the port of New York, in the
State of N.Y., on or about the 18th
day of July, anno Domini 1922; I am
not an anarchist; I am not a polygamist nor a believer in the
practice of polygamy; and it is my intention in good faith to become
a citizen of the United States of America and to permanently reside
therein: SO HELP ME GOD.
Subscribed and sworn to before me in the office of the
Clerk of said Court at Brooklyn, N.Y., this 9th
day of December, anno Domini 1924
